Promoting Healthy Communities with Mobile Health Services
The Community Wellness Program (CWP) is dedicated to improving health access and health equity underserved and at-risk populations across the Southcoast region of Massachusetts and Rhode Island.
The primary mission of the CWP is to provide services to populations with specific needs including identified health risks, and barriers to obtaining healthcare services. Our goal for the communities we serve is to assist individuals in obtaining access to health care via education, screenings, and referrals as indicated or requested.
Services:
Community Wellness:
Services offered to the community include but are not limited to health education and outreach, including offering confidential health screenings, vaccinations, support with pediatric immunization clinics, and point of care testing (POCT).
- Preventative Screenings include Body Mass Index (BMI), Blood pressure readings and Advance Care plan documentation.
- Adult vaccinations include COVID-19, Flu, pneumonia, Hep A and Tetanus
- Point of Care Testing include Hemoglobin A1C, blood glucose and cholesterol readings.
- Health Education topics including women’s health, nutrition/obesity, smoking cessation and chronic diseases such as Cancer, Stroke/Heart Disease, and Diabetes.
- Attendance at health fairs, resources fairs and community events
Street Outreach:
Street Outreach is an initiative within the Community Wellness Program that provides direct street outreach to those who are homeless, impacted by substance use disorder and mental health conditions. Services include wound care, ongoing psych-social supports, and referrals for treatment. We advocate for and to improve the lives of individuals facing complex challenges.
This initiative works collaboratively with a variety of community partners, with the goal of improving access to treatment and long-term wellness.
- Direct services include wound care, ongoing psych-social support, and connection to treatment and resources.
- Health Education topics including harm reduction, substance use disorder, behavioral health, and mental health.
- Available to provide onsite substance use disorder education and Stigma Training to medical providers and professionals.
Meet the Team:
The Community Wellness Program team is made up of a diverse group of individuals who are passionate about serving our communities.
RN Team Leader: Stephanie Perry, RN CARN, Addiction Nurse Specialist and Team Leader of our Community Wellness Program is an experienced and dedicated healthcare professional with a career spanning over 25 years in human service and community street outreach. For the past 15 years, Stephanie has served as an Emergency Room Registered Nurse, providing critical care and support to patients in need.
Stephanie is driven by the goal of reducing the stigma surrounding substance use disorders, mental health challenges, and homelessness while optimizing health outcomes for underserved populations.

Licensed Practical Nurse (LPN): Jennifer Farland has been a community health nurse for over 15 years, with experienced skill set and passion for providing access to quality health care and mental health. Through her work she incorporates a holistic approach, while utilizing education to forge autonomy in establish individual health standards.

Community Outreach Specialists: Tamara Silva is a Certified Medical Assistant for over 5 years of experience, who functions in the Community Wellness Program as an Outreach Specialist. She is fluent in English, Portuguese, and Spanish. Her passions include building meaningful connections and improving wellbeing of our community. She has worked in both outpatient and inpatient settings and enjoys working with a diverse population of patients and is dedicated to providing compassionate care.

Host-Site & Outreach Opportunities:
The Community Wellness Program will provide selected services, education, and outreach opportunities in partnership with a community-based organization, also known as a host-site.
The standard hours of operation of the program are Monday – Friday from 8:00 am to 4:30 pm, with flexibility when requested to provide services during evening hours and on the weekend.

Southcoast Health promotes community health & wellness through the Community Wellness Program, serving people consistent with the Southcoast Health primary service areas. This includes Greater Fall River, Greater New Bedford and Greater Wareham, MA communities.
Southcoast Health Wellness Van:
After 25 years of service and breaking down traditional barrier to care through health screenings, education, vaccinations Southcoast Health has retired the Wellness Van vehicle and transitioned the services to the Community Wellness Program. Southcoast is grateful for the previous team’s dedication to serving the community.
