The Supportive Care Center provides specialized care for patients facing serious or life-limiting illness. At Southcoast Visiting Nurse Association (VNA), we can provide supportive care at any time during an illness, whether the patient is actively seeking treatment or not.
Supportive Care Center, includes hospice and palliative care, is appropriate for patients with any serious chronic or life-limiting illness or combination of illnesses, including but not limited to:
- Cancer
- Heart, lung or liver disease
- Diabetes
- ALS
- Parkinson’s disease
- Stroke
- Multiple sclerosis
- Cystic fibrosis
- HIV/AIDS
- Blood disorders
- Dementia
- Alzheimer’s disease
Southcoast Health VNA’s Supportive Care Center includes an expert team of physicians, nurse practitioners, certified hospice nurses, social workers, chaplains, bereavement counselors, certified nursing assistants and specially trained volunteers.

Our team works with the patient, patient’s family and primary physician to provide:
Symptom Management
We can assist patients with the following symptoms:
- Pain
- Fatigue
- Constipation
- Weight loss
- Weakness
- Shortness of breath
- Difficulty sleeping
- Mental or emotional symptoms (confusion, depression and anxiety)
Supportive Counseling
Our staff will offer advice and guidance on a variety of issues, including treatment options, transitioning from hospital to home and availability of community resources.
Hospice
Hospice is a specialized kind of care that offers comfort and support to patients who are no longer seeking active treatment and have a life expectancy of months or weeks. Hospice is a form of supportive care that aims to improve the quality of life and provide pain relief and other comfort measures for terminally ill people.
Members of your hospice team includes physicians, nurse practitioners, medical director, nurses, home health aides, social workers, mental health providers, trained volunteers and clergy members.
In most cases, hospice is provided in your home, but may be provided in hospitals, skilled nursing facilities and freestanding hospice facilities.

* Alternative therapies, such as massage, Reiki and aromatherapy
* Assistance with medical decisions, e.g. weighing the pros and cons of various treatment choices

We help patients, their families and healthcare teams plan for future and end-of-life care, including assistance with health care proxy, living will, etc.
