Home Health Care

What is home health care?

Home health care is medical care and support services provided at a person’s place of residence. Typical services include skilled nursing, physical therapy, occupational therapy, medical social work and home health aides. Who can receive home health care?

Individuals of all ages with a variety of health care needs can receive home health care services, including those who are recovering from surgery, disabled, or chronically or terminally ill. Home health care is for people who require assistance at home and requires an order from the person’s physician.

How is home health care paid for?

Most home health care services are covered by Medicare, Medicaid and many other insurance companies. Our Customer Care Center staff can help you understand your coverage and will contact your insurance company to determine your specific benefits.

Who provides home health care?

Southcoast Health at Home’s health care services are provided by a dedicated team of registered and licensed practical nurses, occupational and physical therapists and assistants, speech­-language pathologists, medical social workers and certified nursing assistants. We partner with local hospitals, physicians and community agencies to provide the highest quality of care possible.

Supportive Care provides resources, guidance and support for patients and their families coping with serious health conditions.

Are Supportive Care Center services covered by insurance?

Insurance plans may cover all or part of supportive care.

How do I know if I need supportive care?
If you have advanced chronic illness and are at risk of hospitalization, we encourage you to speak to your doctor about receiving supportive care.

Is supportive care the same as hospice?

Supportive Care Center services are provided wherever the patient calls home including hospital, skilled nursing facility or private residence.
