As a patient of Southcoast Health, we want your health care experience to go as smoothly as possible. Easily find the information you need to request medical records.

Please allow at least five business days after receipt of your fully completed form for most routine medical record requests to be fulfilled. Urgent requests should be made by phone. Fees may apply for copies of some medical records.

Patients, did you know you can access and requests your medical records online through MyChart, Southcoast Health’s patient portal?

Medical records are maintained by the Health Information Management Department for all of Southcoast Health. To obtain a copy of your medical record:

  1. Download the Authorization for Release of Information Form in English, Portuguese or Spanish.
  2. Print and complete the authorization form to release medical records.
  3. Mail, email or fax the completed form to:
